The Shoreline Community Care offers services designed for you and your home. With multiple services on offer, The Shoreline Community Care enables you to seek the support that is right for you while having the peace of mind of knowing our team of Registered Health Professionals are available at your leisure. We provide care to those over the age of 65 years old or privately as desired.

Featuring an entirely person-centred care approach to service delivery, The Shoreline Community Care is designed to place yourself and your loved ones in the centre of experience.

We offer a range of services, including:

  • allied health services
  • personal care
  • cleaning
  • shopping support
  • as well as many other services.

Our service delivery model has a strong foundation in the provision of customer service through providing you with choice in all aspects of your life. We believe that your experiences and knowledge shape the direction of your services and supports, which we celebrate with you and those important to you. We recognise that community connectivity is highly important to many people and our services and additional activities promote access to connections outside of the home setting.

We are an approved Home Care Package and Private Services provider. If you don’t already have access to government funding our incredible team will work alongside you in ensuring that the potentially complex process of accessing services is streamlined.

What is the process and what is an ACAT assessment?

To live in a Residential Aged Care Facility, a person must first consult and be assessed by a member of your local Aged Care Assessment Team (ACAT). This is a health care professional in the area of Senior’s Care. The ACAT assessment is provided by the Government free of charge.

An ACAT assessment is an evaluation by a member of your local Aged Care Assessment Team (ACAT). ACAT is responsible for deciding who is eligible for placement in an Aged Care Facility and if you are entitled to received subsidised Aged Care. The assessment involves investigating a person’s mental, physical and social situation.

You may contact ACAT directly by visiting http://www.myagedcare.gov.au for contact details of your local ACAT, or the Aged Care Information Line on 1800 200 422. Your local doctor may also assist you in arranging an ACAT assessment.

Once you have been assessed, the ACAT will provide you with an Aged Care Client Record (ACCR) and you are ready to find a Residential Aged Care Facility that suits your needs and requirements.

If you already have an ACCR, because you were assessed to receive care in your home, please talk to the friendly staff at The Shoreline who will determine if this same assessment is suitable for entry into residential care.

What are the financial considerations?

It is important to understand all the fees involved when accessing home care services. You may wish to talk to an accountant or a financial advisor who specialises in Aged Care Services to receive advice in relation to these costs and your specific situation.

The cost to accessing home care services will vary depending on your individual circumstances. Many clients are eligible for Government subsidies which our staff can help provide you with further information on. The fees & services available are outlined in the Schedule of Services and Fees tab below.

Schedule of Services and Fees

There are four parts to Home Care Package fees:

  1. A package management fee that is deducted from your package amount, which includes the administration and other costs related to managing the funds within your package
  2. A care management fee that is deducted from your package amount, which includes the care planning, registered nurse check ins, clinical assessments and other costs related to providing care oversight.
  3. A client contribution fee, which some providers decide to charge to the client. The Shoreline Community Care does not at this time charge any additional fee directly to the client unless requested by the client.
  4. An income tested fee, as determined by the Government, this fee is applicable to those who have income above a government assessed threshold.

Do we provide Commonwealth Home Support (CHSP) services?

At this time The Shoreline Community Care does not have approval for providing CHSP services due to the upcoming changes expected by the government. Until further information is released on the new community care systems, The Shoreline Community Care can only provide government funded services under Home Care Package system.

Do we support VAD?

The Shoreline supports all its residents to access the care they want; in the manner they choose. The Shoreline supports Voluntary Assisted Dying decisions and enables its residents to access the service as per legislated requirements in NSW.
